Atrium Health Senior Care specializes in treating medical problems affecting adults 65 and older. By understanding how age impacts illness, we tailor the best possible care to members of this aging population.

Our providers offer two options for care – a memory clinic, located on the campus of Atrium Health University, and primary care services to residents of long-term care facilities.

Services Provided at Atrium Health Senior Care

Memory Clinic

Atrium Health Senior Care uses a team approach, which includes a geriatric physician, nurse and social worker, to provide compassionate care to those dealing with memory loss.

The memory clinic, located on the campus of Atrium Health University, is dedicated to the diagnosis and treatment of:

  • Alzheimer's disease
  • Vascular dementia
  • Lewy body dementia
  • Parkinson's disease and dementia
  • Frontotemporal dementia
  • Mild cognitive impairment

The first visit to the memory clinic will take two to three hours, as we walk through a complete history and interview both the patient and the caregiver. At this first visit, patients should be accompanied by someone who knows them well.

We will also:

  • Review all medications
  • Review prior medical records, including lab results, brain scans, etc.
  • Perform a complete physical exam
  • Perform memory testing

Primary Care Services

Atrium Health Senior Care provides primary care services for patients or residents in long-term care facilities in Mecklenburg and Cabarrus counties. Many of our clinicians are certified medical directors or are fellowship trained in geriatric medicine. We provide continuity of care – from the hospital to the facilities in which we practice.

Our physicians and nurse practitioners visit our partner long-term care facilities several days each week and provide on-call services 24 hours a day, seven days a week.

In cooperation with these facilities, we provide:

  • Short-term rehabilitation
  • Orthopedic rehabilitation
  • Long-term care
  • Dementia care
  • Palliative care
  • Comprehensive medication review and management
