Yes. While we have developed a new model for primary care, we are preserving the many benefits that come with being part of a leading healthcare system. These include access to world-class specialty and hospital care, electronic medical records, and the MyAtrium patient portal.

Since Proactive Health individualizes your health and medical care, we believe that just about everyone can benefit from our personalized approach that focuses on six pillars: medical, fitness, nutrition, mindfulness, sleep and social wellness. Our team can help you make the most of our practice offerings and tailor your Proactive Health experience to meet your health goals. We see patients who are at least 18 years old, and there is no upper age limit.

We currently offer a $25 a month and $75 a month subscription. We offer a 5% discount if you choose to pay annually.

Yes. The one-time enrollment fee is $100.

For a minimal monthly fee, you can receive all the benefits of personalized healthcare. After a one-time enrollment fee, Proactive Health patients opt-in to one of the following subscription levels.

Standard ($25) Subscription Benefits:
Health Coach - Our health coaches work with you to develop specific personal health goals, set action plans, and provide continuous support, guidance and accountability every step of the way.

Remote Health Monitoring & Health Chat - Using the Conversa and Fitbit Plus apps, we will help you monitor your health, get reminders about your goals and track your progress from anywhere. You can also make appointments or request refills – all from your mobile device.

Video / Phone Visits - Although next day office visits are generally available, virtual care visits offer an easy way to connect with the Proactive Health team using your phone, tablet or computer. We allow requested family members to participate in your office visits, whether in person or through a virtual visit.

Health Pillar Assessment - Upon enrollment, we will work with you to complete a health assessment that reflects your current medical conditions, health habits and risks.


Premium ($75) Subscription Benefits:
In addition to the above offerings, Premium level subscribers also have access to the following:

90 Day Health Programs - You will have access to 90-day programs for hypertension, diabetes and weight management.

Dietitian Services - Our registered dietitian is available by phone or virtual visit to help you by providing advice and support for lifestyle, medicine and personal health goals.

Proactive Health Patient Events - We offer educational classes and fun patient events on relevant health topics, providing an opportunity to connect, engage and socialize with your care team and other members.

Access to Google+ Online Community - From inspirational recipes and fitness demos to tips for stress management, our online community will provide you with helpful and educational information to keep you healthy.

Monthly Newsletter - A monthly e-mail that features the latest from our online community, the app of the month, exclusive events and so much more!

Your subscription fee covers features that are not covered by health insurance plans. Your insurance will still be charged to cover any healthcare visits and services unrelated to your wellness program – including office visits, specialist visits, hospital stays, lab and diagnostic tests. Copayments, coinsurance and deductibles will apply as usual.

Proactive Health participates in most plans accepted by Atrium Health. Our team can help answer any specific questions you may have.

Yes. Your Medicare Part B coverage will continue to work for services that are currently covered. Your Proactive Health subscription fee covers those services not reimbursed by Medicare. Our team can help answer any specific questions you may have.

Your Proactive Health team will ensure that you receive all recommended preventive services, including an annual wellness exam. We will take it further with a personalized health action plan – which we will create with you and then provide you support, encouragement and accountability throughout the year as you pursue your goals.

We understand that people are seeking value – the highest quality care and health services delivered in the most cost-effective way. Our practice is built to align our incentives with yours to maximize value. Your Proactive Health team will work together to be certain diagnostic tests and referrals are done in the most cost-effective way. Your subscription also includes health services that you might pay extra for elsewhere. We promise to value your time and money as highly as we value your health.

Proactive Health fees cover services which can potentially be eligible for HSA reimbursement for many individuals. We will provide a statement of services delivered upon request for HSA purposes.

Our goal is 100% satisfaction with the Proactive Health experience. You may cancel any time after 90 days if you are not completely satisfied or if your circumstances have changed, and we will happily provide a prorated subscription refund.
