Community Health Workers (CHWs) are professionals who help individuals and families navigate the health system as well as social care programs and resources. At Atrium Health, CHWs work in many different places, such as in a medical practice or out in the community. Their main focus is addressing social factors that affect health. CHWs also provide support to community members through programs aimed at improving health outcomes, access and health equity.

The Community Resource Hub connects Atrium Health providers, patients and other residents of the Charlotte area to a variety of community-based programs and services. The hub allows users to find and choose from thousands of free or reduced-cost resources, including legal assistance, transportation, housing and food.

Project BOOST (Building Optimal Outcomes for Survivors of Trauma) is a hospital-based violence intervention program (HVIP) aimed at providing support to individuals affected by violent injuries, such as gunshot wounds, stabbings, and serious assaults. The program aims to expand services and provide support to patients after they leave the hospital, focusing on mental health, interpersonal safety and overall recovery.

Referral Navigators work alongside the Community Health Worker team to help patients and families with urgent needs. They focus on immediate, short-term needs, manage referrals to other social support services, and connect patients with the right resources. These resources can come from a community-based organization or one of our Case Managers or Community Health Workers for more complex needs. Referral Navigators also work closely with community-based organizations to make sure the Community Resource Hub and referral processes work as well as possible.

Project BOOST serves individuals between the ages of 15-24 who have been admitted to the hospital for treatment of a violent injury. The program helps people experiencing concerns related to interpersonal safety, employment services, housing, mentorship and more.

The program focuses on meeting patients where they are, bringing care into their home to encourage recovery in a safe and supportive space.
Project BOOST focuses on long-term recovery and meeting patients where they are, both physically and emotionally. The program recognizes that many survivors of violence face ongoing safety concerns and emotional challenges when they leave the hospital. By working closely with patients and their families, Project BOOST provides a holistic approach to recovery that goes beyond the hospital, using community resources and case management to help fill the gaps.
Project BOOST partners with community-based organizations to make sure participants get comprehensive support. The program coordinator connects with local community-based organizations to see how they can support Project BOOST participants. These partnerships help expand the services available to patients, including job placement, housing assistance and further mental health services.
Patients who are admitted to the hospital with violent penetrating injuries may be referred to Project BOOST based on their needs and interest. To refer, staff can send a consultation to CMC/Project BOOST in EPIC. Referrals can also be securely emailed to HVIP@AtriumHealth.org. The program is voluntary, and enrollment is based on the patient’s consent and interest.
