Advance Care Planning Class
Directions
Description
Join us in March for an in-person Advance Directive class open to patients and caregivers who want to plan ahead with confidence. You’ll learn what an advance directive is, why a Health Care Power of Attorney (HCPOA) is so important, and how to choose someone to speak for you if you cannot speak for yourself. There will be time to ask questions, and a notary will be available on site so you can complete your documents during the session if you wish. This is a supportive, informative opportunity to ensure your wishes are known and respected. All paperwork will be provided.
