What Is This Service?
This is a Lifestyle Medicine Group Nutrition Visit led by a registered dietitian.
The visit combines healthy eating education, cooking demonstrations, food tasting, and group discussion. The goal is to help you better understand how food affects your health and to teach practical skills that you can use in everyday life.
Some insurance plans require a referral before participation. Depending on your insurance, a referral may be needed from your primary care provider, Lifestyle Medicine provider, virtual provider, or another healthcare professional.
How to Register
Register for the Lifestyle Medicine Group Nutrition Visit on this page by selecting the date you would like to attend.
After you register, our team will review your information and determine whether any additional steps are needed before participation, such as obtaining a referral or collecting additional information.
Once registration is finalized, you will be added to the registered dietitian's schedule. You can also view appointment information and visit details through MyAtriumHealth.
More in-depth event instructions will be sent to the email address used during registration.
Before the Visit
After you are scheduled, you may be asked to complete eCheck-In through MyAtriumHealth, similar to other healthcare appointments.
During eCheck-In, you may be asked to complete nutrition or wellness questionnaires.
These questionnaires help the registered dietitian understand your goals, eating habits, and nutrition concerns so the visit can better meet the needs of the group.
Depending on your insurance and registration status, you may also be asked to complete privacy forms, HIPAA forms, telemedicine consent forms (when applicable), self-pay forms, Medicaid-related forms, or other required Atrium Health paperwork.
