At Atrium Health Care Plus, we’re here to help you. Our team offers coordinated care, helping you get and stay healthier and avoiding unnecessary trips to the hospital or doctor’s office. Coordinated care organizes patient care among the patient's providers and specialists. Managing multiple doctors, multiple medications or multiple appointments can be intimidating. That's why we’re here to deliver comprehensive, team-based, patient-centric care when you need it the most because we want to improve health, elevate hope and advance healing, for you.
Services Offered
Some of the support we are able to provide includes:
- Extended appointment times – usually between 30 minutes to 1 hour, or longer if needed. These extended appointment times allow us to work with patients to create a plan to manage health outcomes when dealing with complex diagnosis. This leads to reduction in readmission and emergency care, ultimately giving patients better outcomes and quality of life.
- Social workers and nurse care managers to help patients stay on track with specialists and their primary care provider.
- Resources to help alleviate “life” concerns like transportation and prescription drug costs.
- An on-site clinical pharmacist to work with patient’s medication management.
- Social rounding at in-patient facilities to help patients stay on track, even during a hospital admission.
- After hours and weekend support to make sure that patients avoid unnecessary readmissions.
Model of Care
You are at the center of everything. Our care team includes a complex care provider, nurse practitioner, social worker, on-site clinical pharmacist and case manager. Together, we work with your primary care provider and specialty provider to provide full, comprehensive care for you. When referring to complex chronic services, some key factors we look for in patients are:
- Three or more hospitalizations in the last 12 months and/or
- Five or more emergency room visits in the last 12 months
- Multiple chronic medical conditions
