Patient Enrollment Process

1. Request a referral from your child's primary care provider. Provider must be within NC or SC
2. Once referral is complete, you can register your child online in CHADIS
3. Complete questionnaires within CHADIS once your child is registered
4. Invite your child’s teacher to complete the teacher initial questionnaire with CHADIS
5. Once you and your child’s teacher has completed CHADIS, please contact the office to schedule your appointment
