Located throughout the Charlotte, NC, region, Atrium Health’s infusion services allow medications to be delivered intravenously (through an IV) or by an injection. Services vary by location, so please contact a specific infusion center to see what’s available.

How to make a referral

To make a referral to an Atrium Health Infusion location, please complete the below referral and medication order forms and submit them by fax to 704-468-3401. For general questions, please call 704-468-3400.
