As we work to continuously improve our program and outcomes, we launched our Perfect Care Initiative in 2019, and a cardiac ERAS® program and high-risk discharge pathway in 2022.
Passive data acquisition through novel integration of biosensors including heart rate, blood pressure, body weight, steps and sleep provides clinicians with advanced metrics for decision making. This is especially important when patients, many of whom live in rural areas, cannot easily return to the doctor’s office for face-to-face visits. Perfect care offers a solution by allowing patients improved access without having to leave home. The ability to learn, reduce barriers and optimize care for our patients outside the brick-and-mortar hospital setting results in improved outcomes.
The Perfect Care Initiative aims to improve patient access and education while eliminating disparities in peri-procedural cardiac surgery care. The initiative proactively engages cardiac surgery patients and their families with a unique, personalized, patient-centered approach to surgery through education, preoperative optimization, standardization of care and remote patient monitoring (RPM).
The initiative creates a periprocedural experience that ensures every patient receives the same level of care, regardless of facility, across the care continuum. Cardiac surgery nurse navigators educate patients during the preoperative phase and we continue care via remote patient monitoring for 90 days post discharge. Utilization of RPM connects patients to their care team via video visits and wearable devices following discharge.
