- Evidence of health insurance coverage. Health insurance coverage must be maintained while enrolled at the College.
- Background Check
- Background and sanction checks conducted prior to official enrollment include but are not limited to the following: social security trace, criminal history, NC Sex Offender Registry, and checks against duly authorized, licensing, disciplining and sanctioning authorities (including the Cumulative Sanction List of the office of the Inspector General). Continuing students will be similarly investigated on a “for cause” basis. Convictions that will specifically preclude final acceptance to the College for all students include, but are not limited to:
- A sex crime
- Exploitation of an endangered adult
- Failure to report battery, neglect, or exploitation of an endangered adult
- Murder
- Voluntary manslaughter
- Involuntary manslaughter within the previous seven (7) years**
- Battery within the previous seven (7) years**
- A felony offense relating to controlled substances within the previous seven (7) years**
- Abuse or neglect of a minor, child or dependent
- Failure to report the abuse of a minor, child or dependent
- Any act that, if it occurred at the organization, could compromise the safety or well-being of patients, employees, visitors, or volunteers of the organization.
- ** Time frames are measured from the date of conviction. In addition, Carolinas College of Health Sciences will not accept any individual:
- Who has abused, neglected, or mistreated a patient or misappropriated a patient’s property, as reflected in the state nurse aide registry, or
- Whose name appears in the N.C. Sex Offender Registry, or
- Who has been convicted of a criminal offense related to health care or who is listed by a federal agency as debarred, excluded or otherwise ineligible for participation in any federally funded healthcare program.
Carolinas College's admissions department will send background check information after acceptance.
Note: According to North Carolina statutes, having a previous conviction of certain misdemeanors or felonies may make the applicant ineligible for licensure and/or certification in selected healthcare fields. Furthermore, conviction of certain misdemeanors or felonies may make student ineligible for placement in certain clinical/fieldwork/practicum sites.
- CPR
- Student must upload:
- American Heart Association (AHA) OR American Red Cross (ARC) CPR certification for Basic Life Support (BLS),
- Advanced Cardiac Life Support (ACLS)
- Advanced Life Support (ALS)
- Pediatric Life Support (PALS).
- Provide a copy of front and back of your E-card or copy of certificate.
- Online BLS courses accepted only with in-person skill test component.
- Only CPR-AED is NOT accepted.
- Drug Screen
- Your appointment MUST be within 45 days of the start of the semester
- A sigh-up link will be accessible through Canvas, after you have been accepted and pay your deposit
- Student must provide proof of a 12-panel drug screening. This screening must include the exact 12 drugs listed below:
- AMP (amphetamine)
- BAR (barbiturates)
- BZP (benzodiazepines)
- COC (cocaine)
- MTD (methadone)
- METH (methamphetamines)
- MDMA (ecstasy)
- OPI (opiates, including heroine)
- OXY (oxycodone)
- PCP (phencyclidine)
- PPX (propoxyphene)
- THC (marijuana)
- Health Assessment
- Your appointment MUST be within 45 days of the start of the semester
- The health assessment form will be accessible through Canvas, after you have been accepted and paid your deposit
- The provider must complete the form.
- You and the provider must sign the bottom of the form.
- TB
- The QuantiFERON Gold TB Test completed in the last 12 months (preferred). Please provide lab report or 2-step TB skin test that has been completed within last 12 months. The 2 step is defined as two separate TB skin tests done within 21 days of each other.
- Two-step PPD is done in a four-appointment schedule:
- Visit 1: Place the first PPD
- Visit 2: 48-72 hours later, the PPD is read.
- Visit 3: 1-3 weeks after Visit 1, the second PPD is placed.
- Visit 4: 48-72 hours later, the second PPD is read.
- If a PPD or QuantiFERON Gold TB Test is positive, please provide:
- Chest X ray (valid for 1 year)
- Individual TB Risk Assessment AND TB symptom evaluation by your physician/HCP.
- Please upload your Individual TB Risk Assessment or TB symptom evaluation under ‘TB symptom screening’ placeholder.
- Enter completion date as 'date read' OR 'result date' .
- For PPD Skin test - Enter expiration date as 1 year from date read.
- For blood test, chest x-ray - Expiration date will be auto-filled as per school guidelines.
- Annual requirement:
- Annually: Quantiferon Gold (Preferred) or 2-step PPD is required.
- If previous positive results: annually, please submit ‘annual TB risk assessment form’ and an attestation form.
- Flu vaccine (Spring admits only)
- MMR
- Measles: 2 doses of measles vaccine administered on or after first birthday, at least 30 days apart.
- Mumps: 1 dose administered on or after 1st birthday
- Rubella: 2 doses of rubella vaccine administered on or after first birthday, at least 30 days apart or Positive MMR titer - Please provide copy of lab work or any document indicating titer results is required (The document should include student's name).
- TDAP
- Student must provide proof of a Tdap (Tetanus, Diphtheria, and Pertussis) vaccination completed in the last 10 years
- Tdap Vaccination must be current throughout the duration of the program
- Varicella
- Student must provide proof of two vaccinations or proof of immunity by titer (ONLY Lab report will be accepted).
- Hep B
- 2-dose series only applies when 2 doses of Heplisav-B are used at least 4 weeks apart. OR 3-dose series required. The interval between dose 1 & dose 2 should be at least 1 month (4 weeks) and dose 2 & dose 3 should be at least 5 months or upload positive antibody titer. Please provide copy of lab work or any document indicating titer results is required (The document should include student's name).
- Meningococcal Conjugate
- One dose before starting 7th grade or age 12, whichever comes first.
- A booster dose before starting 12th grade or age 17, whichever comes first.
- Exceptions:
- Students who received their first dose of MenACWY on or after their 16th birthday are not required to receive a booster dose.
- Students born before 2003 are not required to be vaccinated against meningococcal disease.
