With an enhanced level of care, availability and the latest technology, your child will get the time and attention they deserve from the day they’re born and as they grow up.

Pre-visit forms for well child exams and physicals

For your convenience, Atrium Health Levine Children's Baxter Village Pediatrics will send your age specific pre-visit paperwork via email before your well-child exam/physical. Once you receive the email, please click the link to complete all required forms before your visit. You can easily complete these forms from any smartphone, computer or tablet. Please complete at least 5 days before your visit.

The biggest difference between a subscription-based practice and a traditional practice is size. Subscription-based practices are typically smaller, with fewer patients. See the question below to learn why this can be beneficial.
Because we see fewer patients, our pediatricians can spend more time with your family during visits. This means appointments won’t feel rushed, allowing for a relaxed environment to get all your questions and concerns answered. It also gives us a chance to really know your family and offer highly personalized care for your child.
You’ll pay an annual subscription fee. The subscription fee is not covered by insurance
You may be able to use your HSA to pay the subscription fee. Call customer service for your HSA and ask if your plan may be used for subscription-based care. The customer service number can usually be found on the back of your HSA card.
Yes. We follow all guidelines from the American Academy of Pediatrics, including checkups, immunizations, routine care and sick visits as needed.
Yes. During office hours, you’ll be able to reach your care team by calling or texting. You can also choose the way you want to communicate – by phone, email or secure text.
After office hours and on weekend, your calls will be answered by our after-hours partners, who will provide the expert care your child needs and alert your child’s care team as needed.
Yes. For your convenience, we offer virtual medicine and can connect with you online to determine the best next steps for your child’s care.
Yes. Specialist referrals will be made as needed for your child’s care.
Yes. We accept most medical insurance, and the billing in our practice functions just like any other pediatrician’s office. All costs associated with medical services are billed to your insurance company.
Your insurance will be billed for your doctor’s visit. According to your health insurance plan, you may have to pay a copay or deductible at the end of your appointment.
You can access portions of your child’s electronic medical record online with MyAtriumHealth. That includes things like their medical history, test results and immunization records. You may also get a copy of your child’s medical record at our office.
