Located in Shelby, NC, we offer Cleveland, Gaston, Lincoln and Rutherford County* residents a variety of services to help them feel comfortable wherever they are.

Community Alternatives Program for Disabled Adults (CAP/DA)

Being placed in a nursing home isn’t the only option for care. This program allows Cleveland and other surrounding County residents to receive support services in their own home instead. Participants must be eligible for Medicaid and meet certain conditions for care.

Age Limit: 18 and over

Respite Care

Everyone needs a break. Respite care gives primary caregivers time to themselves while ensuring their loved ones receive the compassionate care they need. The service covers adult day health care and/or in-home aide services.

Age Limit: Caregivers of individuals over 60 as well as other limited criteria

Community Care Management

This program focuses on identifying specific needs and coordinating care for individuals who are referred from the community, hospital case managers, neighbors and churches or who are self-referred.

Age Limit: 60 and over

Options Counseling

The Options Counseling program assists all older adults, persons with disabilities, caregivers or family members who wish to explore available aging and disability services and supports in their community.

Age limit: 18 and over
