Simulated Participants (SP) help with the training and evaluation of healthcare professionals in medicine, nursing, and other programs. A SP is a person who has been coached to accurately and consistently recreate the history, personality, physical findings, and emotional state of an actual patient. They can also portray the role of a family member of a patient or another healthcare professional such as a nurse or technician. Please complete this information form to help us determine how we might incorporate you into our educational activities. This information will be kept confidential.

We will keep this form on file for current and future activities. We will contact you to discuss the program as well as when we have an appropriate role for you. Please keep in mind that all activities are short term and temporary.
