Biddle Point

Atrium Health Biddle Point Family Practice was opened by Atrium Health in 1997 as a community health center serving western and central Charlotte neighborhoods. The facility is home to 9 attending physicians and 9 medical residents. Clinical pharmacists are an integral part of the care team at Biddle Point providing care in a wide range of disease states including cardiometabolic disease, anticoagulation, HIV, asthma, and smoking cessation.

 
   

Elizabeth Family Medicine

Elizabeth Family Medicine is an active teaching facility located in the Elizabeth neighborhood in Charlotte, NC immediately adjacent to Atrium Health Mercy Hospital. The Family Medicine Residency Program is the most recognized component of the dynamic Department of Family Medicine at Carolinas Medical Center (CMC). Department Programs include: pharmacotherapeutic clinics, sports medicine, geriatrics, behavioral medicine, clinical research, community initiatives in primary care, integrative medicine and teaching. The site has both pharmacist-run clinics as well as interdisciplinary clinics where pharmacists serve an active role. Pharmacist clinics include anticoagulation management, diabetes management, hypertension clinic, and HIV adherence clinic. Interdisciplinary teaching clinics where pharmacists serve as teachers and preceptors include HIV clinic and cardiometabolic clinic. Medication Therapy Management (MTM) services are also provided.

   

Myers Park

Located in the Dilworth neighborhood within walking distance to Carolinas Medical Center, this practice site provides continuity care to indigent patients and includes multiple subspecialties. The site has a pharmacist-run anticoagulation clinic and provides pharmacy services including management of diabetes, hypertension, pediatric asthma, HIV and tobacco cessation. Medication Therapy Management (MTM) and adherence services are also provided. Myers Park Clinic also has a 340B dispensing pharmacy to serve the prescription needs of patients receiving care at Myers Park Clinic, Elizabeth Family Medicine, Biddle Point Family Practice, NorthPark Clinic, or those being discharged from one of our 340B eligible acute care facilities. Medical residents are incorporated into the diverse range of services at the site. In the pharmacy, medication synchronization, MTM, and patient assistance services are provided. A specific formulary is closely followed to ensure cost of medications is not a barrier for high risk patients.

   

Managed Care Clinical Pharmacy

The Managed Care Clinical Pharmacy (MCCP) Learning Experience is a longitudinal, mandatory rotation that is completed remotely through the Atrium Health Airport Center. Atrium Health engages in agreements for commercial and Medicare Advantage plans with payers such as Humana, Aetna, Blue Cross Blue Shield of North Carolina and United HealthCare. Residents of the MCCP Learning Experience will be collaborating with the MCCP team and multidisciplinary care teams to promote high quality patient care and lowering overall healthcare cost, while meeting and exceeding quality metrics for Atrium Health’s various contracts. Residents will also be joining the MCCP Clinical Pharmacists to meet with representatives of third-party payers and Population Health leadership to determine areas of need, create action plans, and discuss system-wide initiatives.

 

   

CarolinaCARE

CarolinaCARE, located in Charlotte, is the home-delivery pharmacy for those insured on Atrium Health's employee health and prescription drug plan. CarolinaCARE is also home to the specialty pharmacy dispensing services for any contracted insurers. The pharmacy employs over 30 pharmacists and pharmacy technicians who provide quality care and excellent customer service through a variety of clinical and pharmacy services. Atrium Health's insurance plan covers over 100,000 employees and their dependents in all of Atrium's markets including the Charlotte Metro region, Atrium Health Navicent, Atrium Health Floyd and Atrium Health Wake Forest Baptist. Clinical pharmacists at CarolinaCARE provide chronic disease state management (diabetes, hypertension, dyslipidemia, heart failure, asthma, COPD, etc.) for those on Atrium Health's insurance plan, as well as engage in plan design, benefit investigation, and utilization management.
