Your Proactive Health team will work with you to design an individualized health and wellness program (your My Proactive Health Pulse) tailored to meet your needs. We will assess your overall health with our Proactive Health Pillars Assessment, and then work with you to prioritize and develop a personalized action plan. We can help you develop a simple, GPS-style plan to eat better, improve your fitness, reduce or eliminate medications, improve control of your conditions, get better sleep, or manage your stress.


We will meet you where you are to get you what you need, when you need it. We can see you in the office or engage with you by telephone, video visit, email visit, collaborative care solution, MyAtriumHealth patient portal, and more. If you're not sure what you need, we'll help you figure that out too.


Our providers, health advocates and team are on the health journey with you. We all work together to help you be your healthiest and happiest self – providing accountability, support, information and tools to help you succeed. Greg Weidner, MD, FACP
Medical Director

Dr. Greg Weidner believes in the power of the patient-doctor relationship. Primary care medicine is built upon this relationship – open, transparent, caring, collaborative and continuous. Prevention and total health are central to Dr. Weidner's approach to helping his patients live their best lives. He enjoys helping patients to become confident and empowered to manage their health, and have some fun along the way. Dr. Weidner lives in South Charlotte with his wife and three children, and enjoys music, sports and the outdoors.

Melanie McMinn, MD

Dr. Melanie McMinn believes in actively caring for our community, both in and out of the office. She is an empathetic healthcare provider who strives to empower every patient to make long-lasting, meaningful lifestyle change. She embraces innovative methods of primary care, and strives to keep discussions about preventive care at the center of every interaction. She is also a healthcare provider at a local homeless clinic through the Urban Ministries center. Dr. McMinn lives in South Charlotte with her husband and three young children, and enjoys sports, travel, live music and theater.

Lauren Goeke, FNP-C

Lauren Goeke believes in caring for the whole patient to achieve optimal wellness. She enjoys helping patients learn ways to achieve their optimal health through lifestyle changes and preventative care. Her passion is to continually bring new innovative methods to her patients and empower them to better their lives. She is enthusiastic to bring her non-nursing and nursing experience as part of a collaborative relationship with her patients. Mrs. Goeke lives in South Charlotte with her husband, young son, and their greyhound. They enjoy exploring Charlotte and traveling together as a family.
